Facility:Dayton Children's - Main CampusDepartment:Clinical PharmacySchedule:Full timeHours:40Job Details:The goal of the Clinical Pharmacy Specialist is to promote the safe and effective use of medications by working collaboratively with the pharmacy team, prescribers, and the multidisciplinary health care team within a defined practice area. This position contributes to the goals of the department by providing safe medication therapy to our patients, and identifying, resolving, and preventing drug-related problems in the pediatric setting. Responsibilities include the complete assessment, recommendation, and monitoring of pharmacotherapy for patients ensuring that the highest quality and evidence-based standards for pharmacy practice are met. Additionally, the Clinical Pharmacy Specialist assumes a leadership role in medication management, supporting and participating in the provision of quality patient care, education, and research in the pediatric setting, represents the pharmacy department on multidisciplinary committees, and serves as a pharmacy preceptor to pharmacy residents and students. This position contributes to the goals of the department by being accountable for the development and maintenance of service line specific drug use policy and evidence based, cost effective medication strategies as well as providing clinical services to select patient populations. Along with other members of the health care team, this role is responsible for patient outcomes that meet the high standards of care provided by DCH.

Experience:

Certifications:

  • Licensed pharmacist and eligible for pharmacist licensure in the State of Ohio required

  • PGY1 Pharmacy Residency OR

  • PGY2 Pediatric Pharmacy Residency (or relevant PGY2 specialty residency) OR

  • Board Certified Pediatric Pharmacy Specialist (or board certified in relevant specialty) OR

  • 4 years of pediatric pharmacy experience

Education Requirements:

Bachelors: Pharmacy (Required), Doctorate: Pharmacy (Required)

Certification/License Requirements:

Rph Ohio - Registered pharmacist - Ohio State Board of Pharmacy
