Cuyuna Regional Medical Center * Pharmacy Crosby, MN Professional Patient Care Casual, Professional FTE: Professional/Admininstrative Casual Weekend Requirements: No weekends Posted 05/22/2024 Req # 13158 POSITION SUMMARY Pharmacists will assume responsibility and accountability for the medication treatment for patients and clients. The pharmacist delivers care to the patient/client through a pharmacy medication management system. The pharmacist interacts with the patient/client, care partners and other health team members while maintaining the standards of pharmacy practice. The pharmacist evaluates patient/client medication regimen and profiles it or contacts appropriate caregivers to offer suggestions for change. The pharmacist will be an advocate for the patient/client in ensuring evidence-based medication use is followed. POSITION QUALIFICATIONS Education and Experience: Doctorate of Pharmacy degree or Bachelor of Science. Graduate of an accredited School of Pharmacy. License/Certificates: Current Pharmacist certification or licensure in the State of Minnesota. Special Skills and Aptitudes: Knowledge of pharmacy principles and practices gained through a variety of pharmacy experience. Completion of specific education programs as determined by the department. Knowledge and compliance of state and federal accrediting agencies. Exemplifies self-direction with good organizational, analytical and interpersonal skills. ESSENTIAL RESPONSIBILITIES Reviews prescriptions issued by physicians or other authorized prescribers to assure accuracy and determine formulas and ingredients needed. Compounds medications using standard formulas and processes, such as weighing, measuring, and mixing ingredients. Monitors all posting onto the patient profiles in order that accuracy, drug allergies, drug interactions, duplicate therapy, rational drug therapy and incompatibilities may be realized and better patient care received. Checks patient medication cassettes prior to delivery to patient care areas to ascertain that the correct drug, dosage form, schedule and strength is being dispensed. Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors. Prepares IV admixtures and extemporaneous parenteral products under aseptic conditions. Directs Pharmacy Department personnel engaged in admixing, packaging and labeling pharmaceuticals. Answers questions and provides information to physicians, nurses, patients, and other healthcare professionals on the appropriate use of medications, drug interactions, side effects, dosage, storage, etc. Assists in teaching programs (departmental, medical, nursing, community). Endorses policies and procedures of the department and of the hospital. Ensures the maintenance and proper operation of equipment and supplies in the work areas. Maintains established procedures concerning quality control, security of controlled substances and disposal of hazardous waste. Enters data into the computer system (Pharmacy program), such as patient, prescribed medications and costs, and maintains patient profiles and charge system. Instructs students and other medical personnel on matters pertaining to the Pharmacy Department. Attends and participates on hospital committees such as the Pharmacy and Therapeutics Committee and Performance Improvement Committees. Reviews and monitors drug therapy that requires dosing adjustments as authorized by the physician. Provides pharmaceutical care to all ages of patients from newborns to the elderly. Understand that there are dosage variations between newborn, pediatric, adult and geriatric patients, and knows where to find appropriate information. Treats patients and their families with respect and dignity. Identifies and addresses psychosocial, cultural, ethnic, and religious/spiritual needs of patients and their families. Assists the Pharmacy Director on special assignments. Supervises at least one to two ( -2) members of the Pharmacy Department staff on a daily basis. Assists the Pharmacy Director with responsibilities in accordance with the organization's mission, policy, and applicable laws. Interviews and assists in the hiring process, trains personnel, plans, assigns and directs work, addresses complaints and resolves problems. Demonstrates ability to read, analyze and interpret pharmacy-related journals. Demonstrates ability to respond to common inquiries or complaints from customers, regulatory agencies or members of the hospital staff. Demonstrates ability to effectively present information to management, medical staff, hospital staff and public groups. Demonstrates ability to apply necessary advanced mathematical concepts, such as exponents and logarithms. Demonstrates ability to apply mathematical operations to such tasks as dosage calculations, formulation of pharmaceutical preparations and data analysis. Demonstrates ability to define problems, collect data, establish facts and draw valid conclusions. Demonstrates ability to interpret an extensive variety of technical instructions and deal with several abstract variables. Demonstrate Standards of Excellence when other duties are assigned. Compounded Sterile Preparations: Assures the availability of adequate equipment to safely prepare compounded sterile preparations. Ensures that pharmaceutical preparations are sterile, properly packaged, labeled and stored, and not outdated. Certifies the skills of the compounding staff. Maintains air quality check records. Ensures compliance with USP. Low-risk compounding: The applicable media-fill test will be performed and passed annually, after the initial training and media-fill test is completed and passed.
