The Health Center at Fountain is seeking a full time, experienced advance practice provider to join their team of providers. Located in Fountain, Colorado, this center provides full spectrum family practice care to people who live and work in their community. This site is innovative, collaborative, and cohesive. The position offers great quality of work/life balance with four 10-hour days, no in-patient duties and occasional telephone call backup to the after-hours triage service.

Your new team will include a physician, four advanced practice providers, two RN's, and 1 Behavioral Health provider. Together they provide a team-based approach to healthcare. This position offers an excellent opportunity for career growth, providing a wide variety of opportunities, including process and quality improvement, mentorship, teaching and participating in research.

Our ideal candidate has 2+ years of experience in family medicine practicing in a primary care setting; able to utilize a template based EMR (NextGen); has an unrestricted Colorado license, as well as an active DEA, and is comfortable seeing all ages, and averages 22 patients per day. Must have a strong desire to make a difference in the community. Must embrace a team-oriented approach and be comfortable working with and overseeing Physician Assistants and providing collaboration with Nurse Practitioners. Peak Vista Community Health Centers is a non-profit healthcare organization whose mission is to provide exceptional healthcare to people facing access barriers through clinical programs and education. We provide primary care services (medical, dental and behavioral healthcare) through 22 outpatient health centers in El Paso, Teller, Lincoln, Adams and Kit Carson counties. Our organization has over 800 employees and serves over 81,500 patients in the Pikes Peak and East Central Plains region.

Federal and State funds are available for loan repayment for eligible candidates. Peak Vista offers a full benefits package including medical, dental, vision, life, optional long term and short-term disability and a 403b retirement fund. Additionally, because Peak Vista is a FQHC (Federally Qualified Health Center), the provider’s malpractice insurance is covered through FTCA (Federal Tort Claims Act) government funds.

Peak Vista is accredited by the Accreditation Association for Ambulatory Health Care, Inc. (AAAHC). Compensation (Pay): $117,374.40 to $144,355.71/ yearly, based on experience. Plus a $1258.40 Rural Differential. *Other compensation may include signing bonuses, rural differentials, and relocation reimbursement.
