A Brief Overview The Supervisor of Community Well-Being for University Hospitals Cleveland Medical Center (UHCMC) plays a pivotal role in advancing UH's Community Health Investment (CHI) Strategy. This position not only lifts up the CHI strategy, it aligns with UH's Community Health Needs Assessments (CHNAs) to address the greatest health needs among each of our hospital's communities. This enables UH to ensure our resources are appropriately directed toward outreach, prevention, education and wellness opportunities. The position is essential for meeting the place-based investment strategy of UH Cleveland Medical Center (UHCMC) which focuses on bringing health & wellness services into the communities we serve, ensuring that care is accessible and convenient. The supervisor oversees the planning, development, and execution of community outreach programs at the UH Community Wellness Center at Glenville and UH Otis Moss Jr Health Center, with a strong emphasis on promoting health equity and wellness in the communities surrounding UHCMC. The mission of the UH Community Wellness Centers is to enhance the wellbeing of neighbors within the University Hospitals footprint. This involves building trust, providing health education, implementing wellness programs, conducting health screenings, and ensuring access to care. The UH Otis Moss Jr. Health Center has been serving the Fairfax community for over 25 years as a hub for personalized clinical services and integrated social and economic services. To advance the mission of the UH Glenville Wellness Center and UH Otis Moss Health Center, the supervisor establishes robust relationships between UH leadership, UH community engagement caregivers, community leaders, and residents. Essential Duties Collaborates with the UH Government and Community Relations Department (GCR) specifically the Community Health Manager in the Academic Region to identify priorities and achieve performance outcomes in community health implementation strategies. Coordinates the UH Glenville Wellness Center strategy, ensuring effective design, coordination, and implementation of programs, with a focus on health equity. * Provides support and guidance to a community dietitian, a certified community health worker and a community outreach nurse, to accomplish the implementation of the UH Glenville Wellness Center health strategy on a day to day basis. * Partner with clinical staff and Community Health Manager in the Academic Region to enhance community outreach services and programmatic development at the UH Otis Moss Jr. Health Center. * Responsible for event planning and execution of community outreach events associated with the UH Glenville Wellness Center and UH Otis Moss Health Center. Manages the collection and entry of data to evaluate progress, analyze, and report on community program efforts. Oversees the program operating budget, finances, and accounting for the UH Glenville Wellness Center, adhering to UH policies and procedures under the supervision of the Cleveland Medical Center operations lead and Community Health Manager in the Academic Region. Represents hospital leadership at community and civic events, promoting community outreach programs and collaborating with community partners. Cultivates relationships and builds trust with organizations and individuals aligning directly with UH's Community Investment Strategy to enhance UH's impact.
