Description Seeking experienced Neonatal APP to work in a fast-paced environment, offering an academic health care setting. We offer a variety of shift options, ranging from 10 hour, 12 hour up to 24 hour coverage, including Day/Night, Nights, weekends and holiday hours. The NNP will assess, implement, and evaluate appropriate plan of care for patients. This includes diagnostic and therapeutic interventions, in accordance with the scope of practice of an Neonatal APP and current standard of care. Neonatal APP will manage care for a caseload of medically complex neonates in both the acute phase and transition to home as well as participate in deliveries. Must be able to demonstrate competencies defined in Neonatal APP responsibilities in providing age-appropriate care to neonates from birth to the age of two years. Neonatal APP will be expected to initiate and participate in education, research and professional activities that contribute to improving patient care, Neonatal APP program operation, or the Neonatal APPs own professional development. This position will be a split/shared position within level IV NICU & Stepdown unit at UH Cleveland Medical Center plus community coverage at Tripoint and Ahuja Medical Centers. Responsibilities include: * Identify, assess and coordinate the care of select pediatric in-patients with special health care or rehabilitation needs in order to promote comprehensive family-centered discharge planning in an efficient and cost effective manner. * Identify patient with complex healthcare/rehab needs. * Assess individual patient and patient populations to identify care/education/rehab needs and develops, implements and evaluates programs to meet these needs and coordinate patient care/collaborates with others. * Practices within the guidelines of a Standard Care Arrangement with a collaborating physician. * Actively participates in the quality improvement process. Involved in learning regarding trends in case management. * Initiates and maintains positive relationships with patients/customers. Initiates and maintains positive relationships with co-workers. * Takes responsibility for self-development and supports a learning environment Displays commitment to the mission of the hospital and its values.
