The Incidental Findings Navigator is an individual educated in clinical expertise as an Imaging Professional (Mammographer, Sonographer, Radiographer, MRI, CT, Nuc Med) and/or Clinical Nurse that has acquired training, knowledge and clinical skills in the management of Incidental Findings and the ability to navigate the complex academic health system. The Incidental Findings Navigator will serve as a coordinator for appropriate follow up with patients, their provider and their care team following incidental and/or concerning findings on Radiology procedures/exams. This person provides consultation, collaboration, coordination and support to patients as outlined below.Licensure by the ARRT, NMTCB, ARDMS, or Ohio Board of Nursing as a Registered Nurse. Certification by a nationally recognized professional organization preferred. | Minimum Required: 3 - 5 Years in nursing and/or Imaging Modality.(CLINICAL ONLY): Engages in population appropriate communication. Has knowledge of growth and development milestones and tasks. Gives clear instructions to patients/family regarding treatment. Involves family/guardian in the assessment, initial treatment and continuing care of the patient. Identifies any physical limitations of the patient and deploys intervention when necessary. Recognizes and responds appropriately to patients/families with behavioral health problems. Interprets population related data and plans care appropriately. Identifies and responds appropriately to different needs resulting from, unique psychological needs or those associated with religious / cultural norms. Facilitation of Timely, Friendly, Courteous follow up: Identify patients with incidental findings. Receive and respond to new patient referrals impacted by Incidental findings Functions as a liaison for Radiology, patients, their provider, and care team following incidental findings on a Radiology procedure/exam. Facilitate referrals, consultations and diagnostics. Coordinate care with other team members, if necessary. Coordinate efforts for treatment and promote quality improvement and educational efforts Compiles data and reports data quality measures consistent with organizational needs Ensure patient and family are informed and involved in the plan of care. When able, meet, greet and guide patients during follow up testing. Continuously update patients/family members on progress made on follow up Offers Support to the Discharge Planning Process and Patient/Family Education: Assist as requested to facilitate referrals, consultations and diagnostics. Advocate on behalf of the patient throughout the navigation process. Empower the patient to advocate for their healthcare needs Invite questions that the patient/family may have and arrange for someone to answer those which you are not equipped to answer. Provide on-going support after patients are notified of incidental findings and through periodic phone calls and/or facilitation of office visits and diagnostics. Develops a collaborative relationship with physicians and/or advanced practice providers, and other healthcare providers and/or departments within the hospital setting and in the community Serve on committees relating to incidental findings and participate in appropriate medical center and communit
