Responsibilities

• May respond to incoming calls from referral sources and shares information with the placement staff review home placement and to identify the potential clients' ability to have their needs met in the identified home.• Works in partnership with families.• Coordinates the development and implementation of services/treatment provided to individuals• Is responsible for the development of the Individual Service Plan/Plan of Care in conjunction with the team and funding source.• Oversees all services provided to the individuals and assists in providing oversight to families.• May assist in accessing medical and health-related services for consumers.• Monitors the quality of care delivered to youth in care.• Provides support and education to families on needs and services for the youth on assigned caseload.• Provides monitoring, consultation/training and support to foster families regarding working with individuals receiving services and implementation of service plans.• Makes home visits a minimum of one time per month or as directed based on client needs, with at least one home visit unannounced quarterly.• Will see the individual in placement an additional time during the month which could include either another home visit or community visit.• Responsible for ensuring monthly foster parent documentation/paperwork is received and filed in record by first visit of the next month.• Serves as a liaison between the agency and the service provider(s).• Oversees and facilitates the treatment team.• Has at least a minimum knowledge of and/or familiar with state regulatory standards regarding children in care.• Consults with third party representatives regarding therapy, educational and mental health needs of the youth on assigned caseload.• Develops/maintains a working knowledge of all services/resources provided by the Company and services available within the community. Assists in identifying alternative community service sources.• Maintains relationships with case managers.• Implements/maintains operational processes to ensure compliance with Company policies, requirements and regulatory mandates.• Adheres to and participates in Company’s mandatory HIPAA privacy program/practices and Business Ethics and Compliance programs.• Participates in quarterly growth planning meetings/activities including discussions around staffing and recruitment needs.• Participates in special projects and performs other duties as assigned.

Qualifications

• Bachelor's degree in a human services, counseling, psychology or mental health.• Valid driver's license.• Must have two years of experience with case management or related discipline• Must be able to communicate both verbally and in writing.
