Northeast Washington Alliance Counseling Services WISe Care Coordinator-Chewelah in Chewelah, Washington

*TITLE: * WISe Care Coordinator-Chewelah

DUTIES:  Primary responsibility is to provide Wraparound with Intensive Services (WISe) to individuals through the age of 20 who have intensive and complex mental health needs and their families.  As a Care Coordinator you will facilitate Child and Family Team (CFT) meetings for youth/families and responsible for leading the team through WISe phases and activities during and outside of these meetings. You will be the central point of communications for assigned families and team members.  As care coordinator you identify the strengths and needs of the youth and family and work on building/increasing "natural supports."  Additionally, you provide referral and coordination with services, supports, and team members. Provide mobile crisis outreach services and participate in the provision of crisis response in relation to their current crisis plan and respond to destabilizing events. Provide crisis stabilization by reducing or eliminating immediate stressors and provide counseling to assist in de-escalating behaviors and interactions. Assist youth and families to develop, expand, and implement crisis plans to reflect proactive and graduated strategies. Perform other duties as assigned by the WISe Services Supervisor

MINIMUM QUALIFICATIONS:  Bachelor's Degree in Psychology, Social Work or a related field required.  Must be eligible for at least an Agency Affiliated Counselor Certification with the WA State Department of Health. Two years of experience in direct treatment of seriously emotionally disturbed youth and families is strongly preferred, Must have a high level of flexibility and accessibility. Must have good written & oral communication skills.  Computer literacy desirable -- clinical software used.    Abstract of driving record for past three years required. Satisfactory criminal history backgrounds check results are a condition of employment.

SALARY AND BENEFITS:  Annual salary $61,776.00 -- $82,932.00.  Full time employees (and their families) receive $1100 per month for enrollment in company medical, dental, and vision as well as employer paid life insurance with voluntary plan options, Washington State Retirement Plans and deferred compensation plans, 12 paid holidays per year and start at 12 hours of paid time off per month.
