Southbridge Care Homes Activation Aide Full-Time Permanent in Chatsworth, Ontario

8 shifts bi-weekly

The ideal candidate for our team will have the following qualifications and skills:
Post-secondary diploma in Recreation and Leisure, Therapeutic Recreation, Kinesiology or other related fields.
Restorative Care or similar training. Willing to provide training for the right candidate.
Excellent multi-tasking skills and the ability to work well on a team and independently.
Must possess good communication skills, both written and verbal.
Experience in Long-Term Care or Retirement is preferred.
As an Activation Aide you will be responsible to:
Work co-operatively with the activation team and Physiotherapist to lead exercises.
Perform treatments such as applying ice packs, hot packs and hot wax with direction from the Physiotherapist.
Conduct, document, and evaluate programs for residents.
Monitor and assess residents’ mental, emotional and spiritual health during programming and provide feedback.
Encourage resident participation in programs and activities and assist them in attending (or ensure they are offered, the option of attending), where possible.
Complete all required program and resident care documentation according to department policies and procedures.
Attend in-service education and participate in team meetings to plan, develop and assess resident care plans.
Perform other related duties as assigned.
