The Overlook Intake Coordinator in Charlton, Massachusetts

Position Title:             Intake Coordinator

REPORTS TO:                     Director of Patient Access  

SUPERVISES:                       N/A

DEPARTMENT:                  Central Intake

COMPANY:                          Overlook Visiting Nurse Association

LPN's strongly preferred 

PRINCIPLE DUTIES/ESSENTIAL FUNCTIONS :

 

  • Responsible for collecting home care referral information and entering data into the EMR

  • Assists with phone management and customer service

  • Assists with scheduling on the weekend

  • Assists with following/tracking active clients that have been transferred inpatient

  • Works closely with field community liaisons to track/monitor referral activity

  • Communicates daily with internal staff to obtain necessary data for dissemination to referral sources (branch availability by discipline, territory)

  • This position is full time and the schedule is Monday through Friday with occasional weekend coverage as needed, as well as 2-3 major holidays per year.

  • Maintains confidentiality of patient health information

  • Adheres to OVNA and MHS policies and procedures

    QUALIFICATIONS :

      Must have excellent organizational, communication, and interpersonal skills

  • Computer proficiency required

  • Previous experience with HomeCare HomeBase preferred

  • Knowledge and understanding of medical terminology strongly preferred

  • Licensed LPN in the state of MA strongly preferred

  • Experience in Customer Service or Hospitality preferred

  • Must be able to work in a fast paced environment

  • High School Diploma or equivalent

  • Excellent verbal and written communication skills

  • Ability to read, write and speak the English language

    PHYSICAL REQUIREMENTS/WORKING CONDITIONS :

    Primary work will be performed indoors in an office setting, may be exposed to the elements (heat, cold, rain, snow); walking and standing frequently; occasional lifting of up to 35 pounds.
