As a Case Manager, you'll play a crucial role in providing home-based crisis intervention and case management services to families navigating challenging situations. This full-time position offers a competitive rate of $25 per hour with a standard work schedule of Monday to Friday, 9:00 AM to 5:00 PM, plus flexibility for evening appointments as needed. Youll be based in our Central Islip, NY office and report directly to the Program Director.

What You'll Do:

Crisis Resolution: Help families address and overcome the crises that led to court or Department of Probation involvement, focusing on issues like substance abuse and mental health. Comprehensive Support: Deliver intensive, wrap-around home-based case management and crisis intervention to guide families towards stability. Assess and Plan: Use the Global Appraisal of Individual Needs (GAIN) tool to assess family needs and create strength-based Service Plans. Resource Management: Refer families to partner agencies, outreach to new agencies, and follow up to track progress. Skill Building: Assist parents and youth with behavior management techniques and facilitate educational workshops and recreational events. Documentation and Transportation: Maintain detailed progress notes and case records, and provide transportation for clients to appointments or emergency resources as needed. Team Collaboration: Engage in team meetings, case conferences, and ongoing training to stay current and effective in your role. What Were Looking For:

Educational Background: Bachelors Degree in a relevant field with at least one year of experience working with children and adolescents, or one year of community or social service work with a similar focus. Training Requirements: Participate in 40 hours of specialized training in the first year, covering areas such as Homebuilders, mediation, substance abuse, mental health, and the juvenile justice system. Skills and Abilities: Excellent oral and written communication skills, proficiency in computer applications (e.g., Microsoft Office), and a valid New York State drivers license with access to a personal vehicle. Bilingual Bonus: Proficiency in English and Spanish is highly preferred and will set you apart! What We Offer:
