This role involves providing coordinated care to patients/clients of all age groups, planning and implementing individualized care plans to restore or maintain patient well-being. The Physical Therapist will plan, organize, and conduct physical therapy treatment programs based on medical referrals and their evaluations, ensuring effective coordination of care.

Requirements

Key Responsibilities:

  • Develop and implement individualized physical therapy treatment plans.

  • Evaluate and document patient/client progress, adjusting treatments as necessary.

  • Coordinate care with other healthcare professionals to ensure comprehensive treatment.

  • Educate patients/clients and their families on treatment plans, home exercises, and preventative care.

  • Maintain accurate and timely patient records in compliance with company and regulatory standards.

  • Utilize durable medical equipment as needed and provide training to patients/clients.

  • Ensure patient safety and comfort during therapy sessions.

  • Participate in continuous professional development and stay updated with the latest therapy practices.

Job Qualifications:

  • Education:Bachelor of Science in Physical Therapy from an American Physical Therapy Association approved program.

  • Licensure:Current State License or a multi-state license issued by a state member of the Physical Therapist Licensure Compact (PTLC) if practicing in a state member of the PTLC, or meets one of the requirements listed in 42 CFR §484.115(h).

  • Experience:At least one year of experience as a Physical Therapist in an acute care, rehabilitation setting, or home health preferred.

  • Skills:Proficiency in therapy skills as defined by accepted standards of practice. Strong interpersonal skills. Current CPR certification. Knowledge of durable medical equipment.

  • Transportation:Reliable transportation, a current Driver’s License, and valid and current auto liability insurance.

Environmental and Working Conditions:

  • Work in healthcare facilities or patients’/clients’ homes in various conditions.

  • Possible exposure to blood, bodily fluids, and infectious diseases.

  • Ability to work a flexible schedule and travel locally.

  • Some exposure to unpleasant weather conditions.

Physical and Mental Effort:

  • Ability to perform heavy lifting, bending, pulling, pushing, and prolonged standing and walking.

  • Must be able to address patient/client and family psychosocial needs effectively.

  • Requires good hand-eye coordination and manual dexterity.
