Division: Addiction & Dependency "A&D" Division Reports to: A&D Ahn-Ji-Be-Mah- Diz Recovery Center (ARC) Program Manager Worksite: Ahn-Ji-Be-Mah- Diz Recover Center Work Hours: M-F, 8:00am 4:30pm, varies as needed LLBO Status: Non-Exempt Safety Sensitive: Yes License/Certification Requirements: Valid MN driver's license, current insurance or be insurable is preferred Summary: Provide clients with assistance, coordination, and case-management. Become familiar with clientele released from correctional facilities in the areas of culturally specific substance use treatment, medical and mental health, economic assistance, social housing and life skills advocacy. Duties/Responsibilities: Supervise any grants that are specific to Treatment Coordination. Design a culturally specific opioid use service for Native American Individuals released from a correctional facility, utilizing the existing LLBO resources. Collaborate with treatment team regarding clients. Work with counselors to develop treatment plans for clients. Provide approved transportation for client appointments, cultural and other activities. Develop and implement program procedures such as intake, referrals, treatment plans, and operational hours, workdays and include a plan for outreach to the LLBO Communities. Develop partnerships with state and county correctional facilities and county jails, specifically Cass County, Itasca County, Hubbard County and Beltrami County. Coordinate and conduct interventions and intake referrals for Mental Health. Records data and quarterly reports of program progress. Meet with billing staff to implement funding for activities. Meet with staff in fulfilling grant goals and objectives. Provide active support to client sobriety goals. Attend meetings to inform the public agencies of new program addressing treatment clients. Assist clients with finding housing and employment. Attend and or conduct all mandatory meetings and trainings as required. Ability to Travel in/out of state Understanding and sensitivity to Native American cultural values, lifestyles and traditions. Adhere to ARC P&l) and LLBO GAPP All other related duties as assigned. Physical Requirements: Sedentary work. Subject to internal working conditions. Non-smoking environment. Drive clients to scheduled activities.
