Mercy Medical Center Home Health and Hospice is looking for an organized and driven professional to join us as a Registered Nurse, Hospice.

As a Registered Nurse (RN), you will provide comprehensive skilled care as ordered by the attending physician. The RN educates patients and their family members and ensures the safety of the patient. The RN is accountable for completing accurate documentation and remains compliant with all legal rules and regulations.

  • Provide end of life care in a patient’s home as ordered by the attending physician

  • Observe and monitors patient conditions

  • Individualize plan of care and makes adjustments as needs change

  • Administer medication as prescribed by the physician

  • Manage multi-disciplinary care as applicable

  • Promote continuity of care with appropriate admissions, transfers and discharges

  • Counsel patient and family on the disease/progression

  • Incorporate patient and family in development of plan of care

  • Listen to patient and family members

  • Oversee and supervise total care of patient provided by nurse aides and LPNs

  • Evaluate the treatments and medical condition of the patient according to the plan of care

  • Determine if the level of care being provided meets the patients’ needs

  • Communicate with the LPNs and nurse aides on supervisory visit results

  • Other duties as assigned

  • Registered Nurse with current license in the state of employment; one year of experience as an RN in an acute care setting

  • Hospice experience is desired

  • Ability to create positive impressions and communicate with a variety of people

  • Ability to remain calm, have patience and be accommodating

  • Compassionate and caring while working with patients and families

  • Knowledgeable on nursing best practices

  • Detail-oriented and observant

  • Valid driver's license and auto insurance
