Posting Summary

Serve as Healthcare Navigator ( HCN ) for Supportive Services for Veteran Families ( SSVF ) to assist and connect Veterans to VA healthcare benefits or community health care services where Veterans are not eligible for VA healthcare. Provide case management, care coordination, health education, interdisciplinary collaboration, coordination, and collaboration with the Veterans primary care providers. This position will be located and serve Southern Vermont.

Minimum Qualifications (or equivalent combination of education and experience)

Bachelor degree and two years’ related experience required. Effective verbal and written communications skills and interpersonal skills required. Demonstrated ability to use computerized case management applications required.

Desirable Qualifications

Knowledge of military culture and/or working experience with at risk or homeless population desired. Experience with MS Word and Excel desirable.

Anticipated Pay Range $50,000 - $56,000

Other Information

Special Conditions Position located in different city, Bargaining unit position, Contingent on continued funding, Occasional evening and/or weekends required (if non-exempt position, may result in overtime), A probationary period may be required for current UVM employees, External candidates must complete a 4-month probationary period, Travel to and from worksites required, This position is eligible for full-time telework in accordance with the university telecommuting policy, Background Check required for this position
