Reach-Up Care Manager

Provide integrated service planning, coordination and system navigation for individuals enrolled in Reach-Up that are affected by mental health and/or substance use disorders.

Responsibilities:

  • Navigate systems to increase ability to engage in affective treatment
  • Provide screening and ongoing assessment
  • Administer or refer to inter-agency resources for specialized screening and assessments for substance use, mental health, and trauma
  • Facilitate and monitor treatment plans as appropriate including necessary treatment team meetings
  • Assist to develop, place, monitor and evaluate participant\'s progress in substance use and mental health services as it relates to the Reach Up participant\'s wellness goals.

Requirements:

  • A combination of education and relevant experience working with individuals coping with substance use and mental health issues
  • Knowledge of community resources
  • Valid driver\'s license and use of a personal vehicle

Structure:

  • Full time
  • Non-exempt
  • Union
  • Starting at \$22.75
  • We offer a competitive salary commensurate to experience
