The Arc Erie County New York has been supporting children and adults with disabilities, and their families in achieving their desired quality of life by creating opportunities through comprehensive services for over 65 years.

Our Position: We are currently seeking a full time (40 hrs)Program Managerfor our Day Services program. This position develops an individual’s habilitation plan for each person consistent w/ the individual’s needs, preferences & personal goals. Other duties include but not limited to:

  • Participates in intake meetings for new enrollees & ensures all documentation for the services has been obtained

  • Assesses the individual’s functioning & potential for vocational, self-care, self-direction communication, recreation & social skills to assist in development of staff action plan

  • Maintains monthly progress notes on objectives in the staff action plan

  • Ensures match of individuals to appropriate program location/staffing ratio

Our Schedule: Monday-Friday 8am-4:30pm.No nights, no weekends and no holidays!

Our Rate of Pay: $20-$22 per hour

Our Requirements:

  • Bachelor’s Degree in Rehabilitation Counseling, Psychology, Social Work or closely related developmental disabilities fieldplus(1) yr of experience working w/ people with disabilities.

  • Great communication skills (oral & written!)

  • Ability to supervise activities of staff members & program participants

  • Valid NYS Driver’s License & approved record

Position Summary:

The Program Manager, under supervision, develops an individualized habilitation plan for each person consistent with the individual's needs, preferences and personal goals within the program parameters; the plan is expected to contribute to the person’s current or future capacity for self-determination, integration into the community and independence; provides supervision, to assigned staff, to ensure implementation of services consistent with Agency mission and regulatory requirements.

Reporting Relationship:

Works under the direct supervision of the Coordinator of Day Habilitation.

Job Duties and Responsibilities:

+ + Participates in intake meeting for new enrollees and ensures all documentation for the services has been obtained and is reflected accurately in staff action plan.

  • Assesses the Individual's functioning and potential for vocational, self-care, self-direction, communication, recreation/social skills, etc. to assist in development of staff action plan.

  • Develops individual plan consistent with the Individuals' needs, preferences and personal goals to enable them to live, work, socialize, and participate in the community.

  • Ensures that the staff action plan and Life Plan indicate the name of the Waiver service, the start date, the frequency, the duration, the valued outcomes and POMS information.

  • Minimally reviews the staff action plan every six months to assess relevancy, progress on individualized goals, etc., in order to make changes or modifications as needed. Reviews plan more often if needed or if the Individual, family or Care Coordinator requests a review.

  • Maintains monthly progress notes on objectives in the staff action plan, communicates significant concerns to individual, family, residential provider, Care Coordinator, advocate, etc. following agency policy and procedures.

  • Provides assessment data and records to as requested, assuring adherence to Agency policy and procedures on maintaining Individual confidentiality.

  • Ensures implementation of staff action plan consistent with specified objectives and services.

  • Ensures match of individuals to appropriate program location/staffing ratio

  • With Associate Director of Clinical Services & Associate Director of A16 Clinic coordinates the provision of clinical services for individuals attending the day habilitation program consistent with their needs and desires.

  • Evaluates staff performance consistent with performance evaluation guidelines; reviews written evaluation with appropriate staff member; makes recommendations to Coordinator for promotion, termination.

  • Assesses staff training needs and makes appropriate recommendations.

  • Participates in the recruitment, selection and termination of staff members.

  • Provides orientation of each new staff member to program policies, procedures, reporting requirements.

  • Assists Coordinator of Day Habilitation in establishing and maintaining relationships with community resources to enhance programming

  • Maintains required service, statistical and administrative records for assigned individuals.

  • Participates in staff meetings, group conferences and in-service training; participates in community education activities as requested or assigned by supervisor.

  • Implements an organized program of supervision for Direct Service Professionals

  • Manages activity accounts for individuals enrolled in program

Qualifications and Education Requirements:

  • Bachelor’s Degree in Rehabilitation Counseling, Psychology, Social Work or a closely related developmental disabilities field PLUS one year of experience working with people with disabilities.

PLUS

  • It is essential that the Program Manager have good communication skills – both oral and written. Ability to supervise activities of staff members and program participants is required. Must be able to work in an industrial setting, have manual dexterity, visual acuity in normal range, and the ability to work alone and with others.

A valid New York State Driver’s License and approved driving record. Ownership or access to an insured car in good operating condition is desirable.

The ability to read, write, analyze, be perceptive/comprehensive, show good judgment & decision making skills, independent action, planning & prioritizing and follow directions.   • Human Relations Skills:

The ability to communicate with individuals in a positive manner, demonstrate the ability to serve as an appropriate role model to program participants and co-workers. Receive supervision instruction in a positive manner. Adhere to and administer Agency’s policies, procedures, rules and regulations. Maintain a professional attitude and appearance at all times.

  • Equipment Operation:

The ability to operate household appliances, such as, washer, dryer, stove, etc.; operate office equipment, such as, computer, copier, fax machine, etc.; operate program equipment, such as, lifts, standards, exercise equipment, sensory equipment, etc.; operate agency buses and vans to include use of wheelchair tie down systems; etc.; Use household cleaners, chemicals, and ability to come in contact with normal household dust/dirt.
