Hybrid Schedule 39 Days of PTO per year! $1500 sign on bonus after 90 days of successful employment Position Summary This role is responsible for organizing patient care activities and sharing information among all the participants concerned with a patient's care to achieve safer and more effective treatment outcomes. A commitment to the RHD values should be demonstrated as job duties are performed. Essential Duties and Functions Program Development and Monitoring Be familiar with all regulations for the Program. Build and enhance relationship with RHD program staff and outside providers to coordinate care Maintain up-to-date knowledge and implementation of RHD programs. Through regular, systematic review, ensure thorough analysis of each client's care and ensure that charts are linked with all available resources. Coordination of Care Develop comprehensive treatment plans for each client accessing two or more services across settings. Review compliance of individual treatment plans with outpatient and mobile teams as needed. Maintain tracking system to identify need for comprehensive treatment plans. Interface with in-house and external partner providers through monthly/quarterly off-site visits Facilitate routine comprehensive treatment team collaborative planning meetings. Track and maintain treatment plan compliance for clients in residential treatment. Coordinate transition to outpatient for individuals returning to treatment from a higher level of care and maintaining a 48-hour standard for contact. Support clinical team by liaising with PCPs and other Medical Professionals for coordination of medical care needs. Administrative Duties Provide statistics and outcome/measures on quality of care. Update/respond to weekly and monthly reporting requirements. Takes on personal development and building a learning environment. Participates in training as requested by Supervisor or Director. Qualifications Bachelor's degree in the Human Service field One year experience working with children and/or adults with Mental Health/IDD/Addiction diagnosis Experience working independently.
