Job Summary: As an Advanced Practice Provider, your role encompasses delivering vital healthcare services to diverse populations, transcending traditional clinical confines. Your responsibilities include conducting comprehensive patient assessments, diagnosing and treating various medical conditions, and providing preventive care on a remote telehealth platform. Adept at telehealth, you'll serve patients by addressing healthcare disparities and access to care. Collaboration with healthcare teams, cultural sensitivity, and adherence to state-specific regulations define your practice. This dynamic role empowers you to enhance healthcare accessibility and foster positive patient outcomes.

Key Responsibilities:

  • Patient Assessment: Conduct comprehensive assessments of patient's health conditions, including physical examinations, medical histories, and diagnostic test results.

  • Diagnosis and Treatment: Diagnose and treat common acute and chronic illnesses, injuries, infections, and other medical conditions. Develop treatment plans and prescribe medications as needed.

  • Preventive Care: Provide preventive care and health maintenance services, including vaccinations, health screenings, and lifestyle counseling.

  • Health Education: Educate patients and their families about health conditions, treatment options, medications, and self-care techniques.

  • Care Coordination: Collaborate with other healthcare professionals, such as physicians, nurses, and specialists, to coordinate and manage patient care.

  • Telehealth: Offer seamless virtual healthcare consultations, referrals, and follow-up care through our telehealth platform.

  • Record Keeping: Maintain accurate and up-to-date patient records and documentation in compliance with healthcare regulations and standards.

  • Prescription Management: Manage and monitor patients' medication regimens, including adjusting dosages and renewing prescriptions as needed.

    Requirements:

  • Education: Hold a Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP) degree from an accredited nurse practitioner program.

  • Licensure: Possess a valid and unrestricted nurse practitioner license in the State of New York.

  • Certification: You must be certified as a nurse practitioner by a recognized certifying body, such as the American Nurses Credentialing Center (ANCC) or the American Academy of Nurse Practitioners (AANP).

  • Clinical Skills: Demonstrate proficiency in conducting physical assessments, diagnosing medical conditions, and developing treatment plans.

  • Communication Skills: You must have excellent interpersonal and communication skills to interact effectively with patients, families, and healthcare team members.

  • Flexibility: Be adaptable and able to work within multiple telehealth and adjunctive remote systems to deliver comprehensive and seamless care to patients.

  • Knowledge: Stay current with medical advancements, healthcare guidelines, and best practices through continuing education and professional development.

  • Cultural Competency : Be culturally sensitive and able to provide respectful care to diverse patient populations.

  • Technological Proficiency : Familiarity with electronic health record systems and telehealth technology is required for mobile NPs who offer virtual services.
