Description

TheLicensed Practical Nurse(LPN) assists the Registered Nurse in the implementation of the nursing process on his/her assigned patients. The practice of nursing as an LPN is defined as performing tasks and responsibilities within the framework of case finding, health teaching, health counseling and provisions of supportive and restorative care under the direction of a Registered Professional Nurse. Functions as a member of the overall nursing team to promote, provide and maintain high quality care to patients, in accordance with the New York State Nurse Practice Act.

WHAT YOU WILL DO:

  • Works with members of the medical team in order to provide personalized high quality health care.

  • Ensures patients are prepared to be seen by physicians.

  • Plans nursing care for patients.

  • Assists the physicians and mid-level providers with procedures.

  • Administers medication and/or treatment when indicated.

  • Administers medication therapy and vaccines as ordered by the provider.

  • Maintains and secures all clinical supplies and medication in the service area.

  • Makes follow-up and referral appointments.

  • Conducts patient teaching and counseling; provide patients with health information materials.

  • Makes internal and external referrals for specialties and follows up with their result.

  • Maintains proper documentation in patient’s electronic medical record.

  • Performs and/or assists patients with treatments/procedures.

  • Ensures PCAs are conducting pre-visit planning for all patients daily.

  • Observes, reports and documents significant physical, psychological or safety status changes in patient’s condition to the RN/Provider promptly.

  • Communicates patient care needs to ancillary staff and follow up accordingly.

  • Performs routine screening test as needed, specific to each unit.

  • Maintains an orderly and safe environment and adheres to infection control guidelines.

  • Manages client information following HIPAA guidelines.

  • Conducts an audit of all Oral/Injectable medications.

Requirements

Requirements:

  • Graduate of an accredited school of Practical Nursing.

  • Currently holds New York State License, Certification and Registration.

  • One year of previous related nursing experience preferred.

  • BCLS, Infection Control and Child Abuse certificates required.

  • Bilingual in Spanish or Creole is a plus.

  • must be able to float to all five sites as needed.
