The Role

Under general supervision, the Case Manager is responsible for aid clients requiring social service assistance. In collaboration with clients; interview and evaluate clients and formulate Independent Living Plans. Locate and make use of appropriate community resources for clients; and relocate clients to permanent housing. This work is carried out in support of the mission and goals of Samaritan Daytop Village.

What You Will Do

  • Communicates all aspects of contract requirements to staff.

  • Coordinates, guides and supervises activities of direct reporting staff.

  • Provides direct supervision of Case Managers.

  • Leads and participates in administrative and staff meetings as required.

  • Troubleshoots problems and makes decisions in accordance with program policies, procedures and protocols.

  • Monitors clients’ progress towards their goals and documents via progress notes.

  • Conducts periodic review of all case documentations to ensure accuracy, completeness and clarity.

  • Conducts chart auditing of client files.

  • Directly manages a case portfolio of approximately 25 clients. Participates in monitoring therapeutic environment to ensure that behavioral guidelines are followed, a therapeutic milieu is maintained and interpersonal relationships remain positive.

  • Provides individual counseling and facilitation of caseload group meetings and other assigned groups.

  • Provides psycho-education and counseling germane to chemical dependency and relapse prevention.

  • Administers appropriate behavioral interventions as needed.

  • Participates in assessment screenings to determine client needs/preferences; formulates and implements treatment, transition and discharge/continuing care planning collaboratively with individual clients.

  • Facilitates re-socialization process of clients by serving as a role model. Provides role modeling regarding ethical and professional conduct.

  • Keeps abreast of changes in field.

  • Performs other duties as requested .

    Who You Will Be

REQUIREMENTS:

Education, Experience and Skills Required:

  • H.S Diploma/GED with Five (5) years’ experience preferably supervising Case Managers within transitional/shelter programs or related setting

  • OR

  • Associates Degree is Human Services or related field with at least Two-Three (2-3) years’ experience supervising Case Managers within transitional/shelter programs or related setting

  • OR

  • Bachelor’s Degree in Human Services with at least One-Two (1-2) years’ experience supervising Case Managers within transitional/shelter programs or related setting.

  • Knowledge of CARES a plus.

  • Excellent time management.

  • Experience working directly with people from diverse racial, ethnic and socioeconomic backgrounds.

  • Working knowledge of substance abuse treatment modalities and client self-help/support modalities.

  • Strong writing skills, competencies in writing goal based/person centered treatment plans and progress notes.

  • Computer literacy including proficiency in Microsoft Office Suite and EMR

License/Certification Required:

  • None

    Preferred Education, Certification, Experience and/or Skills:

  • Ability to participate in CRP related trainings

  • Bi-lingual Spanish.
