POSITION OVERVIEW

To assist in the planning, implementation and evaluation of recreational, social emotional and spiritual needs of the resident at Casa Promesa Nursing Facility.

Key Essential functions:

  • Lead groups and individual activity programs as indicated on the monthly calendar.

  • Escort Residents who needs assistance to attend programs throughout the facility.

  • Provided 1:1 activities, supplies or room visits to resident who do not or cannot participate in group's activities due to personal choice or physical, mental conditions.

  • Provided social and sensory situation.

  • Provided emotional support.

  • Adapt programs to the capabilities of the individual residents involved to enable participation.

  • Encourage resident awareness of activities options and encourage participation and pursuit of leisure interest old or new.

  • Interview resident and family members to obtain information regarding past and present leisure of interest.

  • Announce programs throughout the PA system alerting residents of ongoing programs.

  • Show initiative and creativity in programming and working with residents.

  • Prepared supplies and materials needed for programs in advance.

  • Provided inputs to the planning of the monthly recreational calendar according to the needs of the residents.

  • Distribute and place activity calendars in resident's room in monthly basis and as needed.

  • Prepares large calendar and post on their units.

  • Maintain daily participation log of resident's activities.

  • Observed residents attendance, participation, and respond to programs, to evaluate programs and residents progress.

  • Submit monthly statistics to Activities Director on the last day of the month.

  • Assist in developing, implementing and maintaining an ongoing Quality Assurance program for The Recreation Department.

  • Provide refreshments to residents during special events where indicated and complies with residents diets.

  • Log in and Delivered mail in daily basis and assists with reading as requested by residents.

  • Coordinate activities with other departments.

  • Assists in preparing and setting up activities areas for schedule programs.

  • Assist in cleaning up activities area after completion of programs.

  • Accompany residents to field trips when designated,

  • May perform other duties as designated by the Activities Director.

  • Maintained required Initial, annual, quarterly, significant changes and progress note assessments on a timely basis.

  • Develop a comprehensive plan of care for each residents to identified problem/needs/strengths of the resident and the goals to be accomplished and update the care plans on a timely basis at least quarterly or monthly.

  • Ensures that all charted activity progress notes are informative and descriptive of the services provided and the resident involvements and response to the service.

  • Complete MDS on a timely basis as directed by MDS Coordinator.

  • Attends Care Plan meetings as assigned by the Activities Director.

  • Participate in discharge planning process.

  • Develops and maintained good working relationship with other departments within the facility to assure that daily activities can be performed without interruptions.

  • Attend staff meetings to assist inn identifying and correcting areas and /or improvement of services.

  • Recreation leader is responsible for the care and storage of all supplies, material and equipment.

  • Operate equipment in a safe manner.

  • Practice acceptable standards of sanitation, safety, and infection control.

  • Maintain department work area in a clean, sanitary and safe manners.

  • Participate in fire safety and disaster preparedness drill.

  • Report all accidents and incidents immediately.

  • Report all unsafe/hazardous conditions /equipment's immediately.

  • Follow facility procedures when exposure to blood and body fluid

  • Respect resident rights and dignity and practice quality standards of care.

  • Maintains the confidentiality of all residents care information.

  • Review and report residents' complaints ad grievances and initiate investigation and follow up.

  • Assist in training and providing orientation to new staff, internes and volunteers as directed by the Activities Director.

  • Decorate resident's areas for holidays, special events, etc.

  • Attend and participate in training programs.

  • Sits, stands, bends, lifts and moves during working hours.

  • Work in office areas as well as throughout the facility.

  • Communicate with other disciplines.

  • Must work a flexible schedule including evening, weekends and holidays.

  • Must be able to related information concerning a residents conditions.

  • Must be able to cope with mental, and emotional stress of the position,

  • Must function independently and have flexibility, personal integrity and the ability to work effectivity with residents, personals and family.

  • Must be able to work with ill, disable, emotionally upset ad at times hostile residents within the facility.

  • Must be able to push and pull residents in wheelchair.

  • May be exposed to infectious waste, diseases & conditions, including HIV/AIDS and Hepatitis B virus and Covid -19

  • Must be able to assist in the evacuation of residents

QUALIFICATIONS

  • High School diploma or Equivalent

  • Bachelor Degree in Recreation

  • Knowledge of APIE (assessment/planning/implementation/evaluation) program.

  • Minimum of 2 year of experiences as a Recreation Coordinator.
