A mission driven, innovative community health center located in the South Bronx is seeking a like-minded full-time Family Physician or Med-Peds to join the healthcare team serving children, adolescents, and adults.Bronx Health Collective (BHC) is affiliated with Montefiore Medical Center-Albert Einstein College of Medicine.We are seeking a full-time Family Physician or Med-Peds to provide outpatient clinical services to patients of all ages.Our beautifully designed and technologically sophisticated community health centers provide an enhanced medical home with comprehensive primary care in Pediatrics, Reproductive Health, Adult Medicine, and HIV/AIDS care to underserved populations. Mental health and nutrition services are co-located and integrated into primary care. Special health initiatives include Child and Family Resiliency, Prenatal Group and Well-Baby Group, Diabetes, Asthma, Obesity Prevention, and Terra Firma, a medical-legal-mental health partnership clinic for unaccompanied immigrant children and families seeking humanitarian protection. Additionally, workgroups exist for staff to address issues of Trauma Informed Care, Antiracism and Equity, LGBTQIA healthcare and staff wellness. Health educators, case managers, data analysts and public health specialists are an integral part of the health care team.
