Essential Duties:**
Work with a dynamic team of professionals including nurses and social workers in our foster care program for children with special medical needs.
Provide intensive casework services and fieldwork for children and their birth and foster families with the goal of providing permanency as quickly as possible.
With medical staff, team-train foster parents in specialized child care skills.
Conduct intensive outreach to connect children and families with agency and community treatment resources.
Maintain written case records.
Work in partnership with RN assigned to cases; collaboration includes conducting home visits whenever possible.
In-person collaboration is an essential function of the job.
Perform other duties as assigned.
Minimum Qualifications: **
Bachelor’s Degree in Social Work or related field required, Master’s degree preferred.
Foster care experience preferred;
Bilingual (Spanish/English) preferred.
