Essential Duties:**

  • Work with a dynamic team of professionals including nurses and social workers in our foster care program for children with special medical needs.

  • Provide intensive casework services and fieldwork for children and their birth and foster families with the goal of providing permanency as quickly as possible.

  • With medical staff, team-train foster parents in specialized child care skills.

  • Conduct intensive outreach to connect children and families with agency and community treatment resources.

  • Maintain written case records.

  • Work in partnership with RN assigned to cases; collaboration includes conducting home visits whenever possible.

  • In-person collaboration is an essential function of the job.

  • Perform other duties as assigned.

Minimum Qualifications: **

  • Bachelor’s Degree in Social Work or related field required, Master’s degree preferred.

  • Foster care experience preferred;

  • Bilingual (Spanish/English) preferred.

  • Flexibility, and ability to work under pressure and meet deadlines;
