POSITION SUMMARY

The Community Health Worker identifies individuals in the community who are homeless and are not currently engaged in health care services. Fosters strong connections throughout the community with emergency departments and psychiatric units to engage individuals in medical care and behavioral health care services, as needed, prior to their discharge. The goal is to connect individuals with housing, health care, and social supports that will help them maintain stable housing and a reduction in emergency department use. May function as a member of the Mobile Medical Team and provide services on Wheeler’s Mobile Medical Unit(s).

Hourly Rate $18.00

EDUCATION AND EXPERIENCE/QUALIFICATIONS

High School diploma/GED and a minimum of two years of experience working with health care systems and/or the homeless population. Bachelors degree and some experience preferred. Bi-lingual in Spanish is preferred. Valid Drivers License is required. Safely operates vehicle in the community, based on program needs.

    ESSENTIAL DUTIES AND RESPONSIBILITIES

    Provides outreach to traditional and non-traditional settings in the cities and towns covered by the Greater Hartford Coordinated Access Network (CAN) to assist in the identification of individuals who are homeless who need connection to housing supports, medical services, and behavioral health services, and assistance with basic needs.

    Makes contact with individuals who are homeless within the community, at emergency departments and psychiatric units, and SNAP sites to engage in medical and behavioral health services.

    Makes formal treatment referrals for and helps to engage adults in treatment services and recovery supports.

    Collaborates with other providers within the region and develops linkages with organizations that provide services to individuals who are homeless.

    Maintains records of program activities for reporting and tracking purposes.

    Attends program staff meetings and assigned community meetings.

    Responsible for safely driving and operating the Mobile Medical Unit, including all associated operations and maintenance tasks. Tasks may include, but are not limited to operating and/or maintaining the unit’s slide, leveling system, and gas.

    Continues to develop knowledge and understanding about the history, traditions, values, family systems, and artistic expression of groups served as well as uses appropriate methodological approaches, skills, and techniques that reflect an understanding of culture.
