Family Health Centers Community Health Worker Digital Navigator- Bilingual Required in Brewster, Washington

As a member of the Outreach Team, the Community Health Worker provides basic health information from a standard curriculum and assists patients with linkages to health, social and other needed services; assists with outreach events and community networking activities as directed by the Outreach Coordinator. This particular position will focus the basic health information on the digital navigation grant while funding is available.

MAIN DUTIES & RESPONSIBILITIES:

  1. The Digital Navigator aids vulnerable and underserved populations in Washington State bridging the digital divide for healthcare view patient portal, devices, and digital skills coaching. Assistance is primarily in person at clinics, via phone and video calls, and at appropriate events.

    1. Assists with device distribution as outlined in grant deliverables
  2. Assists patients with accessing community resources

    1. Refers patients to community resources and to FHC's full range of health services.
    2. Refers patients to FHC's Enrollment department for assistance in insurance and medical payments
  3. Provides health education to patients via outreach activities

    1.  Assists in coordinating outreach events
    2. Attends outreach events and provides health education in various settings (i.e. schools, orchards, community settings)
  4. Coordinates with FHC marketing on specific marketing initiatives related to Outreach, which include but not limited to flyers, media (banners), advertising, surveys, photographs, radio and newsletters

  5.  Participates in training seminars and/or webinars including health education for lay leaders

    EDUCATION & EXPERIENCE:

    Possesses an intermediate level of written and verbal communications skills in English, has computational and computer skills and mathematical knowledge typically acquired through completion of a high school program. Must have reliable transportation and provide documentation of auto insurance and valid driver's license as required.
