Health First Behavioral Care Manager, LCSW in Palm Bay, Florida

POSITION SUMMARY To be fully engaged in providing Quality/No Harm, Customer Service and Stewardship by providing behavioral health support to the Collaborative Care team, which is led by the patient’s primary care provider and includes a psychiatric consultant. The Behavioral Care Manager is responsible for a patient caseload and will provide evidence-based brief psychological interventions, administer psychological screening and assessments, monitor patients’ progress, and make community referrals when indicated. The Behavioral Care Manager is closely integrated into the Collaborative Care team, with shared responsibility for engaging the patient and coordinating effective treatment.

PRIMARY ACCOUNTABILITIES 1. Support the mental and physical health care of patients on an assigned caseload. Closely coordinate care with patient’s medical provider and, when appropriate, other mental health providers. 2. Provide evidence-based brief psychological interventions such as behavioral activation, problem-solving treatment, motivational interviewing, or other treatments as appropriate. 3. Document patient care, progress, and treatment recommendations in the Athena electronic health record. 4. Update patient information in the patient registry and related software tools to facilitate weekly case review with the consulting psychiatrist. 5. Routinely administer, score, and interpret psychological screening assessments to provide ongoing evidence to support treatment recommendations. 6. Participate in regularly scheduled (usually weekly) caseload consultation with the psychiatric consultant and communicate resulting treatment recommendations to the patient’s medical provider. 7. Complete monthly billing in a timely manner and maintain current knowledge and awareness of payer reimbursement practices and regulations impacting the patient's treatment. 8. Provide patient education about common mental health and substance abuse disorders and the available treatment options.
9. Screen and assess patients for common mental health and substance abuse disorders. Facilitate patient engagement and follow-up care. 10. Maintain and improve clinical skills related to therapeutic methods, with particular emphasis on brief evidence-based treatments. 11. Support psychotropic medication management as prescribed by medical providers, focusing on treatment adherence monitoring, side effects or complications.

MINIMUM QUALIFICATIONS • Education: o MSW degree from a Council on Social Work accredited school or Master's degree in related field with appropriate experience (e.g., master’s degree in counseling psychology) • Licensure: LCSW or equivalent (e.g., LMHC) • Certification: o Current American Heart Association (AHA)BLS Healthcare Provider Completion Card • Work Experience: One (1) year of previous clinical experience in counseling • Knowledge/Skills/Abilities: o Experience with evidence-based brief psychological interventions o Proficiency with Microsoft Office (Word, Excel, PowerPoint, Outlook) o Ability to work independently, including from a remote setting. o Flexibility to manage multiple assignments/priorities and handle a demanding work schedule.
o Demonstrated excellent written, oral and interpersonal communication skills.
o Ability to establish rapport and support patients who may be experiencing psychological distress. o Strong conflict management skills
o Strong critical thinking ability to analyze various situations and identify solutions.

*PREFERRED QUALIFICATIONS * • Work Experience: Three (3) years of previous clinical experience. Clinical experience in a healthcare setting.

PHYSICAL REQUIREMENTS - Sedentary • The majority of time involves sitting or standing; occasional walking, bending, stooping. • Long periods of computer time or at workstation • Light work that may include lifting or moving objects up to 20 pounds with or without assistance. • May be exposed to inside environments with varied temperatures, air quality, lighting and/or low to moderate noise. • Communicating with others to exchange information. • Visual acuity and hand-eye coordination to perform tasks. • Workspace may vary from open to confined, on site or remote. • May require travel to various facilities within and beyond county perimeter; may require use of personal vehicle.
