Over 10,000 homeless individuals are cared for by Boston Health Care for the Homeless Program each year. Job Summary:

NOW OFFERING A $4,000 SIGN ON BONUS

Hours:  Full-time; 40 hours per week. Ability to work 1-2 early evenings per week. Provide rotating on-call coverage for Family Team patients.

Boston Health Care for the Homeless Program (BHCHP) is seeking an experienced Family Nurse Practitioner/Physician Assistant (NP/PA) to join our Family Team which is an innovative program providing high-quality, comprehensive medical services to families and youth/young adults experiencing homelessness.  At BHCHP, the Family Team strives to break intergenerational cycles of homelessness by addressing the acute and chronic medical and mental health needs of our patients with a model of care offering a one-stop shopping for patients with many barriers to accessing traditional medical care.

 In this role, the NP/PA will serve as a midlevel provider on the Family Team and be responsible for the health assessment of families, pregnant individuals, and youth in various sites; individualized care management including health promotion, disease prevention, risk factor intervention; and provision of episodic health care as needed. This role is also responsible for implementing full spectrum primary care as if patients identify BHCHP as their medical home.

Responsibilities:

  • Provide episodic care and primary care to individuals in family and youth shelter programs as well as at our Family Clinic.

  • Work closely with the site case managers to develop, implement, and evaluate individualized care/case management plans based upon a collaborative needs’ assessment.

  • Assist in securing access to primary care and other health care resources for families/pregnant women/youth as needed and assist in enabling smooth transfers of care; facilitate with obtaining clients' health care information as needed.

  • Provide education to patients on health promotion and disease management and prevention; provide on-site health screening as necessary, conduct developmental screening in children seen in clinic, and screen and address social drivers of health; complete forms as required for school, program eligibility, camps, etc.

  • Participate in data collection and analysis for grants, other funders, Family Team, program-wide and advocacy purposes; participate in educational outreach by hosting occasional students and residents in clinic.

  • Collaborate with Family Team members in the development and evaluation of program goals, collaborate with team members in the development and implementation of health education materials/programs as needed in the population served.

    Qualifications:

  • Licensed by the Commonwealth of Massachusetts.

  • Minimum of 1-year experience post-graduation from NP/PA program; pediatric experience is required for this position.

  • Experience working with underserved populations.

  • Must have the physical ability and means to drive to multiple sites; reliable transportation required.

  • Adult outreach experience is preferred.

  • LARC experience preferred.

  • Buprenorphine waivered preferred.

  • Bilingual (Spanish or Haitian Creole) preferred.

  • Must have an understanding of the diverse needs of individuals and families experiencing homeless, including substance use disorders, behavioral health needs, multicultural/immigration diversity, pregnancy/parenting needs, transgender health, and trauma-informed care.

  • Openness to learning new clinical skills to meet shifting needs in the population.
