Job Description

Good Shepherd Fairview Home is looking for a Physical Therapist to join our exceptional Therapy team!

Per Diem hours available. Hourly rate plus an additional 25% for Per Diem!

Position Summary:

The PT evaluates and treats residents who demonstrate disorders of musculoskeletal deficiencies, neurological conditions, balance disorders, and transfer/gait deficits. Evaluation and treatment will only be conducted after a written order is received from the physician.

Essential Job Duties and Responsibilities:

The PT is responsible for the screening of residents of the facility as follows:

  • New facility residents – within 24 hours of admission

  • Long Term residents – upon receipt of a significant change form and/or referral from nursing

  • The PT is responsible for the completion of all necessary documentation, which includes but is not limited to the following items: i nitial evaluation forms, w eekly progress notes, monthly recertification forms and discharge summary.

    Essential Job Requirements

    Education: Doctoral Degree in PT, Masters of Science Degree or Bachelor of Science Degree in Physical Therapy.

License: State Licensed Physical Therapist.

Qualifications: Must be 18 years of age. Ability to read, write and speak the English language

Experience: Preferred working in a long term care facility.
