LPN/ Social Worker Discharge Planner

What You'll Do:

The LPN/Social Worker Discharge Planner will provide appropriate and timely discharge planning from the nursing home, arrangement for follow-up health care, assess financial resources available for patients, and make referrals to appropriate community resources.

Key Responsibilities of a Discharge Planner:

As a LPN/Social Worker Discharge Planner, you will play a critical role in coordinating and facilitating the safe and timely discharge of patients from nursing facilities. You will collaborate with healthcare professionals, patients, and their families to develop comprehensive discharge plans that meet the physical, emotional, and social needs of patients post-discharge. This role requires excellent communication, problem-solving, and organizational skills, as well as a strong understanding of healthcare regulations and resources.

Serve as a strong advocate for residents' rights and preferences, ensuring their voices are heard and respected in care planning and decision-making processes

Connect residents and their families with community resources, support services, and advocacy organizations to address social, financial, and legal needs

What You'll Bring:

Qualifications of a LPN/Social Worker Discharge Planner:

Minimum of LPN Nursing License or Degree in Social Work, Experience in Healthcare Administration, or a related field.

Relevant experience in discharge planning, case management, or a related healthcare field.

Knowledge of healthcare regulations, including Medicare/Medicaid guidelines and insurance coverage requirements.

Familiarity with community resources and support services available for post-discharge care.

Ability to maintain confidentiality and adhere to HIPAA regulations

Excellent communication and interpersonal abilities

Ability to work collaboratively as part of a multidisciplinary team

Empathy, compassion, and a commitment to person-centered care
