The Case Manager is responsible for provision of case management services and crisis intervention for Outreach clients. This individual assists in the full implementation of the program with a special focus on resolving barriers to engagement in services, ensuring linkage to appropriate community-based services to meet client needs, and facilitating integration of care for those individuals and families working with multiple service systems. The schedule for this position is: Monday - Friday 11 am - 7 pm; 12 pm - 8 pm shifts.

Job Scope

Provide case management services in a person/family-centered, strengths-based, recovery-orientated, trauma-informed, and culturally sensitive manner.

Conduct comprehensive assessment of client’s individual and family case management needs.

Plan and coordinate case management needs, including referral and escort to medical, psychiatric, substance use, crisis intervention, legal, housing, education/employment, entitlement, social, and peer support services.

Provide housing support services, including assessing housing needs and preferences; educating about housing options; navigating housing-related benefits; completing housing applications (HRA 2010e); support with housing interviews; advocating for housing placements; support with skills necessary to maintain housing; and developing and sustaining relationships with housing providers.

Ensure coordination between behavioral health providers and other internal and external providers serving each client.

Maintain engagement with individuals who are often disengaged from care, have difficulty adhering to treatment recommendations or have a history of homelessness or involvement in the criminal justice system.

Collaborate with all members of treatment and recovery team to maximize integration of care across all systems and programs.

Facilitate group interventions such as, but not limited to, life skills, vocational skills

Document all interactions with and on behalf of clients in case record

Interfacing with appropriate agencies and systems (DSS, probation, courts, etc.) on behalf of the client and assisting clients in meeting all mandates.

Conducts toxicology tests, as needed.

Submit weekly schedules, ensure prior approval for off-site activities, monitor and enhance effective utilization of time.

Attend and participate in staff/community meeting where appropriate.

Provide crisis intervention and group work services and back up coverage, as needed.

Assist with client transportation needs to and from the site, including but not limited to, driving the agency van on designated and approved route.

Exercise due diligence in the delivery of quality care in line with agency Medicaid Compliance Plan and systems of accountability.

Insure communication through the compliance officer and/or the supervisory structure of any violation of non-compliance with the agency’s Medicaid compliance plan and personnel policy and procedure.

Assists with other programming or grant needs as requested by supervisor.

Qualifications

  • Bachelor’s degree in social work or related field.

  • Minimum 2 years of experience providing direct behavioral health services.

  • Bi-lingual/Bi-cultural preferred.

  • Ability to work effectively in collaboration with diverse groups of people.

    Position Status

This is a full-time non- exempt position.

Work Environment

This job operates in a professional office environment. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines. Must have ability to travel among assigned sites as needed.
