Grace Health Community Health Worker in Battle Creek, Michigan

$18.55+ an hour (with experience)

EXAMPLES OF DUTIES: (This list may not be all inclusive.)

  • Conduct screenings and assessments of patients (and families) to identify needs and connect them to available Grace Health and/or community resources, including the following

  • Food

  • Housing

  • Transportation

  • Economic Stability

  • Neighborhood and build environment

  • Health and healthcare, including prescriptions

  • Conduct assessments of patients and their families to identify needs where outreach, education, and/or engagement is needed. (For example, identify if there has been no care with PCP during current year, complete Health Risk Assessments, review inappropriate ED use, recent hospitalization, gap in routine care, connection to community resources to address SDOH issues). This will be accomplished by using a variety of methods including coordinated text messaging, phone calls, mail and in-person visits at home, other care settings or community events as needed.

  • Coach and motivate patients to engage in primary care to effectively manage their chronic disease or engage in preventive care as appropriate.

  • Actively collaborate with other members of the primary care team including Care Managers, Support Staff and PCPs.

  • Address Social Determinants of Health (SDOH) issues identified during patient contacts and eliminate barriers to care as appropriate.

  • Attend relevant community meetings and represents the interest of Grace Health and its patients.

  • Maintain documentation related to assessment, referral to resources, and coordination with other human resource agencies in the patient chart.

  • Assists with education, and referral for SFD, financial assistance programs, and medical insurance programs.

  • Educates staff regarding community resources.

  • Establishes collaborative working relationships with community partners.

  • Participate in meetings and ongoing training sessions as required.
