Job Description:

Client Monitoring Field Staff Specialist - Alexandria

Hourly Rate $15/hr

Reimbursement for Mileage (Must have your own vehicle and Valid DL)

Hours: 8am – 5pm

They will work directly with elderly persons and adults with disabilities living in our local communities. Assisting individuals with accessing community resources and monitoring delivery of approved services and understanding healthcare, especially in areas related to elderly and disabled adults.

This position requires travel as you will be making 15 in-home visits (per day) to our clients within a region (Alexandria and Surrounding area) approx. 20-80 miles per day. You will be reviewing the approved services client qualifies for and ensuring they are receiving. Training will be in Baton Rouge

Principal Duties and Responsibilities:

  • Prepare for visits by reviewing current LA Plans of Care and other data on participants in OPTS and LOCS Long Term Care Programs to applicants.

  • Comply with routine participant monitoring in-person visits.

  • Complete Monitoring Checklist in LOCS, review provider service logs, and time sheets during in-person visits

  • Document participant changes, outcomes, and any participant issues or complaints

  • Complete Elder Protective and Adult Protective forms, as needed.

  • Route provider issues via LOCS to the Provider Monitoring Unit within one business day

  • Reported change in status via LOCS to the appropriate Client Assessment Specialist within one business

  • Comply with complaint procedures, escalate, and refer complaints to OAAS for Protective Service Services according to OAAS policy.

  • Participate in Continuous Quality Improvement (CQI) process.

Educational, Training, and Experience

  • H.S Diploma or GED required; Associate degree in health or human services, preferred.

  • Valid Louisiana driver’s license

  • Current Louisiana automobile insurance

  • Minimum of one year health related service experience; at least 6 months experience working with older adults or persons with disabilities

  • Knowledge of healthcare, elderly and disabled adult populations preferred.

  • Knowledge of Medicaid preferred.

  • Effective verbal, written, and interpersonal skills.

  • Excellent analytical skills
