Abode Home Health is currently seeking a PRN Physical Therapist - Doctor of Physical Therapy - DPT to support our skilled nursing patients in Denver, CO. In this role, you will support your patients by providing direct care and coordinating with the interdisciplinary team to ensure that both patients and their families have an optimal care experience. Responsibilities

Provide physical therapy services to patients according to a written physician's plan of care Initiate physical therapy program and instruct other personnel and/or family/caregiver members as appropriate Prepare and submit clinical and progress summaries Provide in-service education programs for nursing organization personnel as needed Participate in discharge planning Supervise Physical Therapy Assistants according to policy and state regulations Ensure compliance with all state and federal legal and regulatory requirements Additional responsibilities as assigned

Qualifications

Graduate of a physical therapist education program approved by CAPTE with current licensure to practice in state of operation 2 years of experience in adult physical therapy Home health experience preferred Commitment to clinical and documentation excellence
