Clinical Pharmacist in Dermatology - Ambulatory Care Clinical Pharmacy Services

Location: UCHealth Anschutz Outpatient Pavilion, US:CO:Aurora

Department: Derm Clinic

FTE: Part Time, 0.5, 40.00 hours per pay period (2 weeks)

Shift: Days

Pay: $50.76 - $76.14 / hour. Pay is dependent on applicant's relevant experience

Requirements:

  • Bachelor's Degree in Pharmacy from an accredited school of Pharmacy.

  • State licensure as a Pharmacist and any relevant life support certification as determined at position level.

  • Preferred: Completion of PGY1 residency training with relevant clinical experience.

  • Preferred: Board certification in area of expertise within 1 year of employment.

Summary:

This position is responsible for the development and implementation of ambulatory care clinical pharmacy services and initiatives that optimize the patient care experience and drive organizational performance. The ambulatory clinical pharmacist in dermatology will establish a robust clinical pharmacy practice focusing on patient-centered care and supporting medication management, therapy optimization, and certain operational aspects of pharmacy care delivery related to general and complex medical dermatology.

Responsibilities:

  • Develop and maintain a clinical pharmacy practice in area of expertise with the goal of optimizing safe, effective, and affordable medication management.

  • Performs direct patient care duties related to medication management, consultation, and patient education.

  • Identify, design, and implement improvements in medication use and health care delivery systems to advance patient safety, maximize therapeutic outcomes, and/or control costs with particular focus on biologic stewardship, formulary management, and improving prescription capture.

  • Drive health-system pharmacy initiatives (i.e. biologic stewardship), cost effective therapy, and promotion of start to finish pharmacy care delivery.

  • Maintain accountability for utilization of UCHealth outpatient pharmacies for dermatology medications, focusing on medications requiring specialty pharmacy services.

  • Serve as the medication expert in area of practice for patients, pharmacists, and other health care professionals.

  • Participate in local and system committees with medication-related focus impacting ambulatory care, as appropriate.

  • Precepts pharmacy students and residents.
