We are seeking an Advanced Practice Provider to join one of the nation’s most integrated healthcare systems, with over 1,300 providers in Indianapolis, Indiana.

Highlights

Community Physician Network seeks a Nurse Practitioner or Physician Assistant to join our Palliative Care team in Anderson, IN. The most comprehensive palliative care program of its kind in Indiana, our team is committed to helping patients and families understand where they are on their medical journey and providing the physical and emotional comfort they need.

  • Part-time position; 0.4 FTE/16 hours per week

  • This provider will work autonomously and collaboratively to deliver palliative and health care services to patients in the hospital setting.

  • Focus on comfort measures of patients, end of life care, symptom management, as well as assisting with consults and new weekend admissions.

Qualifications

Ideal candidates will:

· Possess 1-3 years’ Experience as a licensed Nurse Practitioner in Palliative Care (strongly preferred)

  • Be comfortable assessing patients independently

  • Be compassionate caregivers with excellent organizational, communication, and critical thinking skills

  • Possess highly-developed interpersonal and group dynamic skills, as well as proven skills as a consultant/educator to friends, families, and loved ones

Licensure & Certifications

Educational background consistent with requirements for licensure and certification within the state of Indiana
