Job Requisition: REQ2623 Employment Type: Full-Time Shift: Days Hours Per Week: 40 hours

Hiring Range : $18.48 - $24.03

The actual compensation for this position will be determined based on experience and other factors permitted by law.

Responsibilities: • Identifies potential clients, completes initial needs assessment, and maintains follow-up to ensure appropriate guidance to health services.

• Provides informal counseling, education, and social support to clients. Assists clients with utilizing available resources, including scheduling medical appointments, accompanying clients to appointments, and assisting with completion of medical program applications.

• Develops and maintains strong working relationships with external (referral sources and community agencies) and internal contacts (departmental team members).

• Maintains accurate client records by documenting all contacts, services provided, and outcomes in order to track all clients and services for future reference.

Education:

• Bachelor's degree required with 1 year experience in Mental Health field.

• Must be able to speak, read, write and follow instructions in English.

Complexity of Work: • Within scope of job, requires critical thinking skills, decisive judgment and the ability to work with minimal supervision. Must be able to work in a stressful environment.

Personal Protective Equipment: • Follows Standard Precautions using personal protective equipment as required.
