Hilltown Community Health Center Health Access Navigator in Amherst, Massachusetts

AMHERST Health Access Navigator The Health Access Navigator will provide comprehensive health access, case management, and referral services to Massachusetts residents. Responsibilities include informing Hilltown, residents about publicly funded health and social service programs and assisting clients in determining eligibility and submitting applications when appropriate. The Health Access Navigator will also act as a resource for HCHC staff for information about programs available to assist patients/clients in need of health insurance or other social service support programs and collaborate with other HCHC providers to enable patients/clients to access needed resources. What we need from you: Certified Navigator strongly preferred, but not required Certified SHINE representative strongly preferred Experience providing rural outreach preferred. Strong MS Office skills preferred. Experience with an Electronic Medical Record (Epic) preferred Bilingual English & Spanish strongly preferred.
