Corus Health Home Health Physical Therapist - Northeast ABQ, East Mountains in Albuquerque, New Mexico

Nominated by our own employees, Corus Health is the proud recent recipient of Albuquerque's prestigious Top Workplace award. As a full-service hospice and home care agency providing a broad range of services to our customers, we are currently seeking a Full Time Physical Therapist to help our patients regain independence by providing therapy services in their home environments primarily in Northeast Albuquerque and occasionally in the East Mountains. Under the Home Health Director's supervision, our therapists perform patient assessments and implement individualized plans of care to help our clients achieve their functional goals.

Responsibilities & Duties:

  • Performing home visits to evaluate patient's level of function

  • Developing an individual plan of care based on the client's personal goals

  • Implementing the treatment program through direct treatment of the patient

  • Reassessing clinical signs and symptoms to determine effectiveness of program

  • Instructing patient, family, and interdisciplinary team members regarding participation in the treatment program

  • Ability to effectively supervise Physical Therapist Assistants as part of the interdisciplinary team

Requirements

Skills & Qualifications:

  • Degree in physical therapy from a university accredited by the American Physical Therapy Association

  • Home health experience strongly preferred

  • License to Practice by the State of New Mexico

  • Comprehensive knowledge of the principles, concepts, and methodology required of an physical therapist and its practice application in the treatment of hospice patients at home

  • Excellent Communication and documentation skills

  • Willingness to work with a interdisciplinary team

  • Valid New Mexico driver’s license and reliable, insured transportation
