Corus Health Home Health Clinical Manager (RN) in Albuquerque, New Mexico

As a full-service home care & hospice agency providing a broad range of services to our customers, we are currently seeking a highly qualified Registered Nurse with proven leadership skills. Under the supervision of the Director of Home Health, the Home Health Clinical Manager is primarily responsible for effectively managing the clinical staff of our Home Health Division. Duties & Responsibilities:

  • Supervise the planning, coordination and delivery of the interdisciplinary group interventions

  • Assign patient/family to appropriate staff member to ensure quality of care and patient satisfaction

  • Participate and provide leadership in the team for regular interdisciplinary group meetings; the development, review and revision of the patient/family plan of care; facilitation of problem-solving sessions; and delivery of team support

  • Implement and monitor compliance with policies and procedures

  • Serve as a member of the management team in order to address problems associated with the delivery of services and staff development needs, to put forth suggestions for appropriate additions/changes in policies and procedures and to give peer input to enhance and improve the management function

  • Supervise the maintenance of medical records assuring accuracy, completeness and compliance with licensing regulations, certification standards, legal and ethical standards

  • Provide supervision of support and clerical staff in cooperation with other members of the management team

  • Responsible for direct management of all clinical home health staff members

  • Conduct appropriate supervisory visits which allow for clinical observation of performance of assigned team members

  • Provide information for inclusion in written performance evaluations for all staff assigned to team in cooperation with appropriate members of the management team

  • Provide leadership and counsel to measurably enhance staff development

  • Identify necessity and assist with staff education and develop in-service programs for interdisciplinary group

  • Participate in orientation of new personnel as appropriate to team responsibilities

  • Teaches, supervises and counsels the Home Health patient and family members about providing care for the patient as needed

  • Performs specific nursing procedures as needed (e.g., treatments, management of symptoms) following doctor’s orders

  • Always communicates to the Director if unable to meet a patient’s need or perform a procedure

  • Audits clinical documentation to ensure quality regulations and measures are being met

  • Delivers competence-based programs for staff with a focus upon improvement of skills and retention of knowledge

  • Prepares short/long-term planning and programs to address identified needs

  • Develop customer satisfaction improvement programs and provide effective education for all clinical staff in order to improve star ratings

  • Actively works to improve patient engagement in health care planning and delivery

  • Maintain on-call schedule for division

  • Assume responsibility for evening, weekends, and holidays on call duty as scheduled and consistent with the policies and procedures of the agency

Requirements

Qualifications & Requirements:

  • Current, unencumbered RN license in the state of New Mexico

  • 2+ years of proven management experience preferred

  • 2+ years of experience working in home health environment required

  • Active patient contact within past three years

  • Ability to supervise others appropriately with sound judgement, clarity, and without bias

  • Adaptable and accepting of various social, racial, cultural and religions

  • Ability to respond appropriately to crisis in a post-acute setting

  • Effective communication skills with a proactive approach to providing guidance and support to staff
