As a full-service home care & hospice agency providing a broad range of services to our customers, we are currently seeking a qualified and compassionate Admissions Nurse (RN) to initiate starts of care for our home health patients. Responsibilities & Duties:

  • Evaluate and treat patients using the most current technology and practices

  • Communicate with the physician to decide the initial plan of care based on initial patient assessment & physician’s orders

  • Successful management of a consistent case load

  • Participate in clinical outcomes monitoring, follow up, and agency performance improvement initiatives

  • Manage and educate Home Health Aides and LPNs performance in implementing nursing services

  • Ability to work in a collaborative environment with an interdisciplinary team

Requirements

Skills & Requirements:

  • Current and unencumbered license to practice as a Registered Nurse in the state of New Mexico

  • Minimum of one (1) year of home health nursing experience preferred, but not required

  • Current CPR certification

  • Reliable transportation, valid driver's license with no DUI's, revocations, or suspensions in the last 5 years

  • Exceptional customer care

  • Excellent written and verbal communication skills

  • MS Office skills (Word, Excel, PowerPoint)
