The Residential Habilitation Coordinator/QIDP will supervise and oversee the provision of services for individuals, as determined by the Interdisciplinary Team. The RHC/QIDP will function as an advocate for the individuals we support and reports directly to the RHC/QIDP Supervisor.

Job Specifications:

  • Participate as a member of the Interdisciplinary Team (IDT).

  • Coordinate the Plan of Care for assigned individuals; schedule and facilitate Interdisciplinary Team meetings, prepare reports and submit in a timely manner.

  • Notify families, correspondents and staff regarding IDT meeting schedule. Prepare Individual Staff Action Plans while conducting meetings semi-annually.

  • Respond to clinical issues immediately.

  • Assist in training staff on implementation of goals/services for assigned individuals.

  • Serve as and maintain contact with appointed guardians and family members.

  • Evaluate and update as needed the various service plans (DDP2, IPOP) and ensure annual updates.

  • Review and coordinate with care management for Life Plan updates and ensure accuracy within the plan.

  • Conduct site observations of programs implementation; ensure the implementation of service plan; assist to ensure residence are following guidelines correctly i.e. Dining plans, behavior plans.

  • Prepare and oversee implementation of residential services.

  • Serve as advocate for assigned individual by helping to identify valued outcomes or personal goals.

  • Supervise the delivery of services according to agency policy, state and federal regulations. Participate and/or be available for the surveys.

  • Assist in developing behavior plans/guidelines for the individuals on your caseload with the agency clinical department.

Requirements

  • A Bachelor’s Degree in human service related field and a minimum of one (1) year experience working with people with ID/DD (QIDP status) and a valid NYS driver license.

  • Must have the ability to function as a team leader. Must demonstrate good verbal and written communication skills and the ability to coordinate the overall planning and implementation of individual programs plans. Must have good organizational and problem solving skills.
