Description

As a Licensed Practical Nurse, you will coordinate appropriate medical care, dental care, and mental health services nursing care/services to a youth and adolescent population with emotional, behavioral, and physical needs. You will ensure that medical treatment services meet the requirements of the regulating agency. You will report to a Registered Nurse and you will work onsite.

Responsibilities:

  • Complete the nursing observations report findings to the registered nurse

  • Review lab and test results, reporting critical values to RNs and providers

  • Medication management including ordering medications and obtaining prior authorizations

  • Dispense medications

  • Communicate with medical director/providers regarding the observation in the client’s health/behavior

  • Provide client and family education regarding medications and treatments

  • Collaborate with our staff regarding concerns and behaviors

  • Participate in Interdisciplinary Medication Review meetings

  • Nursing documentation using EHR

  • Update discharge planning needs

  • Other duties as assigned within the scope of practice and based on agency needs

    Requirements and Education:

  • LPN Licensed in New York State

  • 1 to 2 years of experience

  • Experience working with youth and adolescent populations desired
