CHC Addiction Services CDCA Case Manager in Akron, Ohio

CHC Addiction Services is a non-profit social service agency in the Akron area whose mission is to treat, inspire, support and empower individuals and families impacted by the disease of addiction.

We are currently looking for a Case Manager for our Outpatient program. The Case Manager directs patients through substance abuse treatment through interactions with both CHC Addiction Services and in the community.

 

Duties include:

  • Provide community outreach and information about behavioral health and addiction treatment services and access to care to individuals, groups, and other potential referral sources
  • Facilitate client engagement in treatment by assisting with barrier removal and coordination of care with referral sources and external entities
  • Completing appropriate case recording requirements
  • Communicate with patient treatment providers
  • Provide patient access to individualized treatment
  • Promote patient participation in outreach activities
  • Present patient cases weekly in clinical supervision meetings and seek additional professional consultation as needed 
  • Will assist with group coverage when needed

Minimum Requirements: Bachelor's Degree (Relevant professional experience may be considered in lieu of a degree),  CDCA or other license or credential that conforms to OhioMHAS regulations pertaining to counseling services, and clean driving record.
