ProMedica Health System Coordinator Education and Programming- Social Determinants - Hickman Hospital in Adrian, Michigan

The Charles and Virginia Hickman Hospital will give our community its first-ever wellness destination. Our families and friends will have improved access to nutrition education, exercise resources and state-of-the-art care like they never have before. We offer over 200,000 square feet and has 58 acute cate beds in our new state-of-the-art regional hospital. Our campus also features an organic farm with access to nutrition, education and therapies. We will serve as a community hub that touches residents of all ages and promotes a higher quality of life. This is a new model of collaboration between ProMedica and the community. We are excited to set a new standard for health and wellness in Lenawee County.

REPORTING RELATIONSHIPS/SUPERVISORY RESPONSIBILITIES

The Coordinator Resilient Communities Education & Programming reports to the Director Community Impact, Michigan and is a member of the Social Determinants of Health Team. This position coordinates the day-to-day operations and programming to ensure residents have access to technology, literacy programming, and other resources empowering them to reach their highest potential for health.

POSITION SUMMARY

The Coordinator Resilient Communities Education & Programming leads the day-to-day operations and programming at the Adrian Ebeid Neighborhood Promise’s Technology Center. The coordinator works with the Adrian Resilient Communities Manager and Director of Community Impact, Michigan to deliver programs and resources designed to address health disparities and inequities in east Adrian.

ACCOUNTABILITIES

Coordinates the administration, operations, and outcomes of the programming at the Adrian Ebeid Neighborhood Promise Technology Center and in the community. Assist in assessing prospective learners and registered tutors, coordinates scheduling and tutor matching. Consults with staff from the Adrian REA Literacy Center to develop and implement programming for youth and families in east Adrian and surrounding neighborhoods. Works with the Director Community Impact, Michigan to recruit tutors to facilitate the literacy initiatives Works with Adrian ENP staff to gain and sustain volunteer support, utilizing the talents and resources volunteers bring to the program. Maintain, monitor, and report program data, records, progress reports and follow-up to Adrian ENP staff. Perform other duties as assigned.

REQUIRED QUALIFICATIONS

Education: High school graduate or GED. Experience in public health, healthcare, Social Determinants of Health, education or other related fields.

Skills: Self-motivated individual that establishes positive relationships with residents, team members, and community partners. Actively listens and demonstrates compassion, teamwork, excellence and innovation.

Proven experience using Microsoft Office Suite and Google Classroom. Demonstrated ability to build trust and relationships with the community that will enhance the care provided to youth and families. Demonstrated experience working with diverse populations, youth and families. Strong verbal and written communication and presentation skills. Ability to assess data and information to identify unfavorable health, environment or economic trends that impact youth and families.
