CAREGIVER (FORMERLY DAYBREAK COMMUNITY SERVICES INC) Direct Support Professional (Abilene) in Abilene, Texas

Essential Functions:

  • Validates the quality delivery of residential services by Direct Support Professionals to ensure the health and safety of individuals served
  • Provides direct care to individuals receiving services such as cooking, grooming, bathing and toileting as needed
  • Ensures DSPs are trained in individual treatment and training plans in partnership with Residential Services Director
  • Submit grocery list to Office Manager weekly, ensuring a well-rounded and nutritious diet for individuals. Maintain supplies needed in assigned group home
  • Ensure individual dietary and texture needs correlate with doctor's orders for all individuals

Position Summary: The Direct Support Professional (DSP) is responsible for providing daily residential support services, education, and transportation for the Home and Community-Based Service (HCS) and ICF-IDD program in a supported home living or community-based setting.

The DSP is expected to perform all job responsibilities in alignment with the mission and core values of the Caregiver organization and comply with all policies and procedures, OSHA, CMS, HIPPA, local, state, and federal, regulations.

Essential Duties and Responsibilities:

  • Provide direct client care, including but not limited to:

  • Assist with daily living activities such as grooming, eating, bathing, dressing and personal hygiene

  • Administering medications as directed
  • Assist with meal planning, preparation, and general housekeeping duties
  • Provide appropriate therapy (i.e. physical, behavioral, and occupational)
  • Assist with ambulation and mobility as needed, which may require lifting 50 or more pounds at a time

  • Facilitates inclusion in community activities, use of natural supports, social interaction, participation in leisure activities, and development of socially valued behavior

  • Responsible for preventing client abuse by adhering to all policies and procedures
  • Reports all incidents/injuries in a timely manner and cooperates with all internal/external investigations as required
  • Attends periodic training, reporting suspicious and inappropriate behavior, and following mandatory abuse reporting requirements
  • Provide client transportation to employer, doctor appointments and other community activities
  • Complete administrative documentation including daily notes detailing client activities supporting the approved Individual Support Plan (ISP)
  • Maintains good work and personal ethics in dealing with co-workers and resident consumers
  • Other Duties as assigned

Education:

  • High school diploma or GED required

Qualifications:

Valid state driver's license and driving record that meets minimum standards for insurance coverage under the company's coverage

Clear background check, including criminal history

Ability to read, write, speak, and communicate effectively in English

Ability to perform basic computer functions to complete documentation electronically

Ability to comprehend instructions, manuals, and other company documents

Understands basic mathematical concepts of addition, subtraction, multiplication, and division
