CareCoach Connect Community Health Worker delivers a comprehensive model that plans/implements culturally sensitive and effective in-home family outreach services, while ensuring that services provided are in alignment with department requests and services. The CHW is responsible for providing outreach education services and basic information regarding clinic and community services to promote and encourage visits. The CHW will engage with a team of clinical and administrative staff to determine patient needs.

Primary Responsibilities:

  • Engages members primarily face to face or over the phone to discuss their need for provider follow up

  • Creates a positive experience and relationship with the member

  • Proactively engages the member to manage their own health and healthcare decisions

  • Provides member education on community resources and benefits

  • Listens actively, communicates with empathy, and gathers information in a respectful manner

  • Conducts outreach to encourage participation in health-related programs, services, and/or providers

  • Engages patients by driving to the homes of the unable to reach population by trying to get a provider visit

  • Utilizes methods that promote learning and positive behavior change

  • Uses a variety of interactive teaching and coaching methods for different learning styles

  • Utilizes the appropriate material to leave behind to the patient’s home

  • Prepares and distributes educational materials and presents at community events

  • Performs all other related duties as assigned

Required Qualifications:

  • Texas driver’s license

  • Ability to attain CHW certification within 1 year of hire

  • 2+ years of experience within the community

  • Ability to maintain and contribute to a safe working environment

  • Ability to work with others as part of a team

  • Ability to travel up to 75% around the Abilene and Wichita Falls, Texas area to meet with members in their homes, the hospital or in a long-term care setting

  • Currently reside in the northwest Texas area

Preferred Qualifications:

  • High School diploma/GED

  • Current Certification as Community Health Worker by the State of Texas

  • 2+ years of experience in the delivery of service that are resident-driven and work with families to address multiple issues in-home and/or community-based environment

  • Bilingual(English/Spanish) (read, write, and speak)

Physical Qualifications:

  • Ability to lift up to 50 pounds

  • Ability to push or pull heavy objects using up to 70 pounds of force

  • Ability to sit for extended periods of time

  • Ability to stand for extended periods of time

  • Ability to use fine motor skills to operate office equipment and/or machinery

  • Ability to properly drive and operate a company vehicle

  • Ability to receive and comprehend instructions verbally and/or in writing

  • Ability to use logical reasoning for simple and complex problem solving
