SUNY Upstate Medical University Colorectal Surgical RN Navigator in Syracuse, New York

Job Summary:

The Colorectal Surgical RN Navigator is a Registered Nurse whose clinical nursing experience guides patients and their families to informed decision-making. Assumes responsibilities and accountability for planning, guiding, and evaluating the delivery of patient care. Monitors patient's progress throughout treatment. Intervenes as needed to ensure compliance and optimum care. Collaborates within a multidisciplinary team to facilitate timely, appropriate screening, diagnosis, therapeutic intervention, treatment, and supportive care across the healthcare continuum. The RN Navigator assumes responsibility for the coordination of all resources necessary to provide care. Utilizes advanced expertise to identify, implement, and improve processes to provide an integrative model of care to the patients, while assuring the highest level of patient satisfaction. The RN Navigator provides patient education for each patient regarding their disease process and treatment throughout the healthcare continuum. Demonstrates and promotes professional development. Maintains a professional, safe, and therapeutic environment for rendering care. The RN Navigator will work with the colorectal team to establish policies and procedures and coordinate in the Cancer Center. Will also assist in the application process to achieve National Accreditation as a Rectal Cancer program.

Minimum Qualifications:

NYS licensed/registered or eligible RN required. Bachelor's degree in Nursing or healthcare related field three (3) years post licensure care RN experience (preferably in specialty) OR an AAS in Nursing and five (5) years post licensure RN experience (preferably in specialty) required. Excellent written/oral communication and interpersonal skills required. CPR-BLS certification required by the time of appointment.

Preferred Qualifications:

Three (3) years as a Certified Nurse/Patient Navigator in a specialty area preferred.
