Job Description

Aramark SeniorLIFE+ is seeking a Clinical Dietitian to join us at our Grove of St. Charles senior living community located in St. Charles, IL. The ideal candidate would have a passion for providing nutritional services to the elderly.

Job Responsibilities

  • Perform medical nutrition therapy assessments on residents in order to address acute and chronic conditions.

  • Educate residents on therapeutic diets and nutrition interventions to improve outcomes and quality of life.

  • Work collaboratively with nursing, dietary manager, speech language pathologist, physicians and nurse practitioners to deliver appropriate individualized nutritional therapy including therapeutic diets, oral nutrition supplements and texture-modified diets.

  • Monitor and assess residents with weight changes, enteral feeding, dialysis and wounds.

  • Participate in resident review meetings to address high-risk residents.

  • Conducts patient nutrition assessment on a population of varying levels of acuity patients

  • Manages the development and implementation plan for nutrition intervention in accordance with the Nutrition Care Process

  • Provides nutrition counseling for individuals, taking into consideration any adaptations to teaching methods necessary to meet patient learning needs

  • Utilizes assessment and evaluation techniques that consider the varied needs of age-specific populations as well as cultural, religious, and ethnic concerns

  • Provides appropriate follow-ups in accordance with the patient?s treatment goals, referring patients for outpatient counseling, community, or home health services as appropriate

  • Works cooperatively with medical staff as well as food and nutrition services staff to ensure conformance to prescribed nutrition care orders

  • Provides education that ensures compliance with food safety, sanitation, and overall workplace safety standards

  • Provides appropriate and timely documentation that summarizes the nutrition care plan in a patient?s medical record, including nutrition assessment, diagnosis, plan, implementation, and progress towards goals

  • Demonstrates accountability for the proper use of patients? protected health information

Qualifications

  • Bachelor's degree required, Master's preferred in Dietetics or related field

  • Registered Dietitian (RD) or Registered Dietitian Nutritionist (RDN) credential

  • Illinois state licensure as a Dietitian

  • Familiarity with PointClickCare and MDS process is a plus
