Provides quality nursing care to residents in an environment that promotes their rights, dignity, freedom of choice, and individuality. - Demonstrate respect, attention, and awareness toward the diversity among the people we serve (residents, families, caregivers, etc. (through their attitude, service, and actions. - Is knowledgeable of the individualized care plan for residents and provides support to the residents by their care plan. - Identifies and participates in the QAPI process or leaves as an area of development. - Assists in daily requirements and tasks in the care and treatment of residents. Areas of care include bathroom assistance, general personal hygiene (bathing, dental hygiene, hair and nails) dressing, mobility assistance and food intake, and other needs in keeping with the individuals\' care requirements. - Maintains resident rights and confidentiality. - Documents all ADLs in electronic health records. - Observes and reports symptoms, reactions, and other changes in condition promptly. - Routinely turns and positions residents as required by the care plan. - Complies with all aspects of residents\' rights. Is responsible for promptly reporting any incidents or evidence or resident abuse or violations of resident rights to the charge nurse or administration. - Honor the resident\'s right to refuse care and report such refusal to the nurse supervisor. - Assist in maintaining a safe, neat, and clean environment. - Answers resident call bells promptly and courteously. - Ensure that residents who are unable to call for help are checked frequently. - Takes and documents temperature, pulse, and respiration rates, food and fluid intake and output, and heights and weights and records in EHR. - Ambulates and transfers residents using the appropriate assistive devices and body mechanics. - Use only the equipment you have been trained to use, operate more safely, and report any defective equipment to the supervisor immediately. - Communicate and interact effectively and tactfully with the residents, visitors, families, peers, and supervisors. - Assist and escort residents to appointments such as the hair salon or attending activities, church services, or doctor\'s appointments. - Complies with all CMS & DPH, and JCAHO regulations, including safety, infection control, and standard precautions procedures at all times. - Attend and participate in all mandatory, and scheduled training and educational classes to maintain current certification as a Nursing Assistant. - All responsibilities will be conducted in a manner that exhibits the BHCS mission, vision, and core values.
